The purpose of this questionnaire is to guide you in planning your next step. It will also help to better advise you when you have your personal coaching session. Be as honest and complete as you can.
NOTE: Some of the questions are likely to require extended time to answer. We strongly suggest you write ALL your answers in a separate text document that you can keep safe on your computer and return to until you are finished writing. When done preparing your answers, you can then copy and paste each answer into the appropriate text box in the web form.
